NAC Entry Form?

Does anyone know where the USFA has the NAC entry form on their website? I looked in the obvious place under forms and could not find it. It's not in the document section either.

You're right. It's odd.

There is usually a spot in the forms download area for NAC entries. It's not there.

There are also NAC entry forms generally available in the schedules area for the various NAC competition information lists. There aren't any there, either.

The form as originally created was incorrect. It had a registration fee of $30 and an event fee of $50 (rather than the $40/$40 that it should have been). I pointed out this error to Michael Massik a couple of weeks ago, the delay could easily be that they went to post and realized that the error had not yet been corrected. One would hope that it'll be up soon (given a reg. deadline of what, 3 weeks from now? Get your entry in early and avoid busy fax lines. :) ).

Incidently, at least some copies of the erroneous form have been distributed, so be careful about that.

This is funny. I asked the same question via e-mail to the usfa and Christine Simmons sent me a pdf of the form. It has the $30/50 fees on it. If those prices are wrong, then I have the wrong form.

If you know the prices to be correct at $40 for each event and registration, I can change the form and send it to whoever needs it.

The prices $40/$40 are correct. It was $40/$30 last year and the BoD raised it to $40/$40 for this coming season. The change was improperly done when new forms were made.

They posted the incorrect form at the website again. I just check today and it shows the $30/50 schedule.
