Tourn insurance for rented gym

We plan to rent a gym (a YMCA facility) in order to host our upcoming tournament. They gym owner requires a proof of insurance and some paperwork I have to fill out.

I know USFA will provide coverage for that, but my question is, how do I go about getting insured? Is there a form at the USFA website? Do I have to speak with someone directly? How do I get the proof sent back to me so I can show it to the guy at the YMCA?

Yes, there is a tournament venue insurance form. Contact USFA directly to ask, if you can't find it in the forms and documents page. Look in the club membership form page. There's an additional event insurance form. You don't need to be a club to get event insurance. It costs $50 for a one-time event.
