USADA Abbreviated Theraputic Use Exception Form

Usfencing.org seems to be down again and I can't find the TUE on international.usfencing.org. Trying to get it for a doctor's appointment this afternoon.

Two questions:

1. Does anybody have an alternate location for this form? I can get *a* form from the USADA website, but the instructions there say that many sports have specific forms.

2. Looking at Google's cache of the usfencing.org forms page, it appears that there is a different form for 2005. Between the 2005 U.S. Fencing form and the 2006 USADA form, does anyone know which is the correct one?

For those that may need it in the future, U.S. Fencing uses the generic form from the USADA found at:

While some of the sporting NGBs have specific forms, so far, I've never seen fencing in the "need to get a different form from the standard TUE" column.

But each search may be different.

The important part is to file early, and keep copies of everything, including any return communications. Keep them in your bag for any meet at which you might be drug tested.

If you're in the "out of competition" testing group, keep an up-to-date folder handy for that 8 a.m. doorbell ring!
