Faulty Summer Nationals entry form

Just sent a query of to the National Office, but perhaps someone here has an insight.

On the first page of the Individual Event Entry Form it asks for the entrant to "List the events you are entering from the worksheet on page 3."

But there is no worksheet on page three of the entry. Page 3 of the entry is the fax-back Confirmation Form.

Have you considered using the "USFA Summer Nationals Event Worksheet" on page 4?

Well it does look like someone added "worksheet" way up at the top of page 4, but it sure looks like a schedule, and it is named a "Schedule," and there's no codes on it to use for entering on page one (like the codes we use on the NAC entries). So I'm guessing there was some miscommunication in the assembly of these forms.
