Try days and see where you get. Escorting prisoners to the bathroom is a short term solution. You have to ensure, each and every time that they go, that they have left nothing for another prisoner to use, that they are secure and that the transport itself is conducted in a safe and orderly manner.
Further, if you do tell a prisoner to "hold it", what's he going to do? I'll tell you. He'll piss out the bars of his cell, preferably on a guard. Or he'll make a nice puddle right in front of the door, so that you have to step in it everytime you open the door to get him for "piss breaks". And what about lock-downs? Where you want to control the prisoner in a highly secure cell for days on end? What then? It's not possible to do isolation either, unless, of course, you want him to have to sleep in his own feces and urine.
THAT is considered torture.
